Register as a New Patient

Before filling out a new patient registration form and questionnaire, please check our practice catchment boundary by using the map and postcode checker opposite. Alternatively, call 01527 575800 or contact us online to check your eligibility.

To register with this surgery:

  • Fill out a new patient registration form and a new patient health questionnaire via the links below (your registration will be declined if both forms are not completed)
  • Visit reception and collect a paper registration pack

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

All Patients are given a named, accountable GP who is responsible for coordinating your care. You can still talk to or make an appointment with any of the clinicians, not just your named doctor. You can request a particular GP and reasonable efforts will be made to accommodate your preference.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Registering as a Patient Proxy

If you would like to apply for Proxy Access, please contact us. You will be asked to provide proof of ID which will be retained in the clinical record.

You can use linked profiles as a parent or carer to access online health services in your own NHS account, using the NHS App on behalf of other people through proxy access.
